Post-traumatic pneumocephalus

چکیده
A 79-year-old male presented to the emergency department after a facial trauma because of a mechanical fall 3 h prior with no history of loss of consciousness. The patient had Parkinson’s disease and atrial fibrillation (AF), and was on treatment with acetylsalicylic acid for AF. The family reported that he was dependent for all basic daily activities and had mild cognitive deterioration. Physical examination showed that he was lethargic, but oriented, bradykinetic, and had gait and balance difficulty. No headache or confusion was reported. All cranial nerves were intact, and there were no motor or sensory deficits. He had superficial lacerations in the nasal and frontal regions. Nasal and ocular orbit X-rays were taken, showing nasal fracture and pneumocephalus (Fig. 1). Computed tomography scan confirmed pneumocephalus and multiple craniofacial fractures (Fig. 2). After a week of observation and prophylactic antimicrobial therapy during which there was no clinical deterioration, the patient was discharged. At follow-up 5 weeks later, the patient was at his clinical baseline. Repeated imaging showed progressive resolution. Pneumocephalus is the presence of air in the cranial vault. It is usually associated with neurosurgery, barotrauma, basilar skull fractures, sinus fractures, nasopharyngeal tumor invasion and meningitis [1, 2]. Headache and altered consciousness are the most common symptoms [3]. Tension pneumocephalus can occur and is a neurosurgical emergency [4]. Plain X-rays can diagnose pneumocephalus, but CT scan is the diagnostic modality of choice [5]. A classical CT sign of tension pneumocephalus is the “Mount Fuji sign”: the massive accumulation of air that separates and compresses both frontal lobes and mimics the profile of the large volcano in Japan. In our case, the air only compressed the right frontal lobe. Most cases of pneumocephalus resolve spontaneously, and conservative management should be provided. Communicating Hydrocephalus Onset Following a Traumatic Tension Pneumocephalus
The entrapment of intracranial air from the check valve system results in a tension pneumocephalus. It should be distinguished from simple pneumocephalus because they are intracranial space-occupying masses that can threaten life. Communicating hydrocephalus is a serious and frequent complication of post-traumatic head injury. Head injury is one of the most common causes in etiopathogenesis of ...
متن کاملOTOGENIC TENSION PNEUMOCEPHALUS CAUSED BY THERAPEUTIC LUMBAR CSF DRAINAGE FOR POST-TRAUMATIC HYDROCEPHALUS: A CASE REPORT Otogenic tension pneumocephalus caused by therapeutic lumbar CSF drainage for post-traumatic hydrocephalus: A case report
Tension pneumocephalus occurs when a continuous flow of air accumulates in the intracranial cavity and produces a mass effect on the brain. We describe a case in which tension pneumocephalus was caused by the performance of continuous lumbar CSF drainage in a middle-aged man who had experienced a temporal bone fracture. Continuous lumbar CSF drainage is commonly performed in patients with tempo...
متن کاملPost-traumatic epidural tension pneumocephalus: a case report
INTRODUCTION Pneumocephalus is usually a self-limiting condition commonly associated with neurosurgical interventions, head and facial trauma. In contrast, tension pneumocephalus is extremely rare, and considered a neurosurgical emergency. CASE PRESENTATION We present a rare case of post-traumatic epidural tension pneumocephalus in a 30-year-old white man who deteriorated rapidly after a blun...
متن کاملPost-Traumatic Tension Pneumocephalus: Series of Four Patients and Review of the Literature.
Tension pneumocephalus is an uncommon and life-threatening neurological condition. It requires emergent and immediate attention to prevent fatal complications. Head injury is the most common cause of tension pneumocephalus. Air can gain access into the cranium either through a fracture involving paranasal sinus or the middle ear cavity or even more rarely in association with a compound depresse...

OBJECT The purpose of this study was to compare the efficacy of the prophylactic use of ceftriaxone for the prevention of meningitis in patients with acute traumatic pneumocephalus. METHODS In this prospective, single-institution, randomized clinical trial, 109 patients with mild head injury and traumatic pneumocephalus were randomly assigned to receive or not receive an antibiotic medication...
متن کاملTraumatic Pneumocephalus
Pneumocephalus is defined as an intracranial gas collection which can be localized in the epidural, subdural, subarachnoid, intraventricular or intraparenchymal spaces. Pneumocephalus has numerous causes, including barotrauma, neoplasms, and spinal anesthesia. However, the majority of pneumocephalus cases are traumatic or iatrogenic in nature. A review of 295 patients with pneumocephalus by Mar...
